ABSTRACT

OBJECTIVES@#This study evaluated the incidence of colorectal cancer (CRC) according to the number of metabolic syndrome (MetS) components.@*METHODS@#Using health checkup and insurance claims data of 6,365,409 subjects, the occurrence of CRC according to stage of MetS by sex was determined from the date of the health checkup in 2009 until December 31, 2018.@*RESULTS@#Cumulative incidence rates (CIR) of CRC in men and women was 3.9 and 2.8 per 1000 (p < 0.001), respectively. CIR of CRC for the normal, pre-MetS, and MetS groups in men was 2.6, 3.9, and 5.5 per 1000 (p < 0.001) and CIR in women was 2.1, 2.9, and 4.5 per 1000 (p < 0.001), respectively. Compared with the normal group, the hazard ratio (HR) of CRC for the pre-MetS group was 1.25 (95% CI 1.17-1.33) in men and 1.09 (95% CI 1.02-1.17) in women, and the HR of CRC for the MetS group was 1.54 (95% CI 1.43-1.65) in men and 1.39 (95% CI 1.26-1.53) in women after adjustment.@*CONCLUSIONS@#We found that MetS is a risk factor for CRC in this study. Therefore, the prevention and active management of MetS would contribute to the prevention of CRC.

Background@#According to the previous studies, the work-related accident rate decreased in Korea after the introduction of occupational health and safety management system (OHSMS), but there were several disasters in Korea such as subway worker's death at Guui station in 2016 and the Taean thermal power plant accident in 2018, which escalated the social demand for safety. In 2018, OHSMS became an international standard, as ISO45001 was announced. @*Methods@#A survey was conducted to research the implementation status of OHSMS and changes in people's perception, and the results were compared with those of a past survey. @*Results@#Enhanced social demand and various stakeholders' (not only buyer) needs, and social responsibility are perceived as the motivation for the introduction of OHSMS rather than legal compliance or customer demand. In the questionnaire about problems with the implementation of OHSMS, the factors with higher response rate in 2018 than 2004 were “excessive cost” and “complicated documentation management.” In the questionnaire about how to promote OHSMS in organizations, most people answered “reduction of workers' compensation insurance rate” in 2004, but most people answered “exemption from health and safety supervision” in 2018. @*Conclusion@#For the effective implementation of ISO45001, emphasis is placed on social demand, training to recognize health and safety as a part of management, and the reduction of certification and consulting costs to promote the introduction of OHSMS. Incentives such as insurance premium cuts and exemptions from health and safety supervision are needed.

ABSTRACT

PURPOSE: As a follow-up for the validity study of Community Health Surveys (CHSs), the purpose of this study was to evaluate the factors affecting the accuracy of CHSs by investigating subjects' characteristics. MATERIALS AND METHODS: We used data from 11,217 participants (aged 19 years or older) who had participated in the CHS, conducted by a local government in 2008 and analyzed the variables affecting the sensitivity and specificity of hospitalization and outpatient visit. RESULTS: Multivariate logistic regression analysis showed that, factors related with the sensitivity of hospitalization and outpatient visit questions were gender, age, marital status, chronic diseases, medical checkup, the subjective health status and necessary medical services. Factors related with the specificity were gender, marital status, educational background, chronic diseases, medical checkup, alcohol consumption, necessary medical services and sadness. CONCLUSION: This study revealed the subject-related factors associated with the validity of the CHS. Efforts to improve the sensitivity and the specificity from self-report questionnaires should consider how the characteristics of subjects may affect their responses.

ABSTRACT

Human immunodeficiency virus (HIV), when infected causes various systemic changes and is the cause of the fatal disease called acquired immune deficiency syndrome (AIDS). With the sharp increase of infection in women, pregnancy can be complicated by HIV infection. Although there have been astonishing developments in therapy there are still many risks in the pregnant HIV patients. The anesthetic management will depend on the patients clinical status, current treatment as well as the well-being of the fetus. We report a case of spinal anesthesia in a 36-year-old pregnant HIV patient undergoing cesarean section.

ABSTRACT

The purpose of this study was to investigate whether the health status as determined by a self-assessment checklist was related to the nutritional status of elderly Korean women. We interviewed 59 female subjects living in the Daegu area aged over 55 years. Information on their dietary intake on two consecutive days was collected by the 24-hour Recall Method. Their body compositions were measured using a Bio-electrical Impedence Fatness Analyzer. Their zinc status was evaluated according to their dietary zinc intake and urinary zinc excretion in urine samples collected for 24 hours. Their heath status was determined by a NSI Checklist. While 50.9% of the total subjects belonged to a moderate nutritional risk group, 23.7% of the subjects were at a high nutritional risk. It appeared that only 25.4% of the subjects were evaluated as being healthy. Therefore, the overall nutritional health status of the elderly females belonged to the moderate nutritional risk group. Except for vitamin C, the average daily nutrient intake of the subjects was below the level of the recommended dietary allowances (RDA) for Koreans. The lower the nutritional health score, the better was their dietary intake of energy (p < .05), protein (p < .001), and zinc (p < .01). A positive relationship between their nutritional health score and body fat ratio was also observed (p < .05). Therefore, we suggest that the NSI Checklist could be used as a practical screening tool at the beginning of nutritional surveillance of the elderly in the community.

ABSTRACT

In an attempt to figure out the relationship between zinc status and taste acuity of old and young women, dietary zinc intake, urinary zinc excretion, and taste acuity were determined for 118 women. Zinc intake was measured by 2-day food records and food frequency method. Urinary zinc excretion was measured from urine samples collected for twenty four hours. Body fat, lean body mass (LBM), and total body water were measured by bio-impedence. Average dietary zinc intake by food record was 4.15+/-1.33mg (=35% if Korean RDA) for the old women and 5.41+/-2.76mg (=25% of RDA) for young women. When zinc intake was measured by a frequency method, the average intakes of the old and young women were 3.5+/-1.7mg 4.5+/-1.9mg, respectively. It appears that dietary zinc intake of young women was significantly higher than that of the old women. Average urinary zinc excretion of the subjects was 0.27+/-0.16mg in the elderly and 0.24+/-0.13mg in young women, which indicated a marginal zinc status. However, zinc status was not significantly different between old and young women. Correlation analysis indicated that zinc intake and urinary zinc excretion were positively related to BMI and LBM in young women. The old women (m=49) showed significantly higher taste detection thresholds than young subjects (n=47) for both sweet and salty tastes. Recognition thresholds for sodium chloride and sucrose were not significantly different between old and young women. The lower the taste thresholds for salty taste, the higher the average dietary zinc intake. However, taste perception concentration was not related to the urinary zinc excretion level.
